Arizona Coalition on Aging – AZCOA is dedicated to advocating and sharing resources for the Valley’s senior population. Focused on being a hub for like-minded individuals and organizations, AZCOA fosters a collaborative environment perfect for networking and generating new ideas. Membership is free of charge and open to any who provides services to the aging population. The organization, as it is now, was helped off the ground by our Director, Dustin Jackson, who is still a very active presence in meetings.

Encompass Health Valley of the Sun – Encompass Health is the nation’s largest provider of inpatient rehabilitation. Their team has decades of experience and a deep commitment to personalized care. Their services include three hours of therapy each day, 5 days a week, along with 24/7 nursing care.

Reunion Peoria – Reunion is accredited by the Center for Improvement in Healthcare Quality and recognized as a Center of Excellence in Rehabilitation and Stroke Rehabilitation. Amenities include hi-tech therapy gyms, an ADL suite, a car transfer simulation, and an outdoor courtyard for relaxation and therapy activities. Each patient receives an individualized care plan of intensive, results-oriented therapies, supervised by an expert team of medical rehab professionals.

Vintage Silver Center – The Vintage Silver Center offers recreational day programs for seniors in Phoenix. Their mission is to enhance the well-being and quality of life for seniors through a holistic approach that integrates recreational therapy, support groups, professional development, and access to vital resources. Each member receives a recreational therapy plan tailored to their strengths and interests to achieve their goals.

Hospice of the Valley Dementia & Education Campus – Located in Phoenix, the Dementia Care and Education Campus, an innovation from Hospice of the Valley, is a place where families can find exceptional care, education, and support in one place.

Innovation Care Partners – ICP is an organization focused on transforming healthcare delivery in the local Scottsdale community. They are a clinically integrated network and Accountable Care Organization. Their emphasis on clinical integration improves healthcare delivery by improving outcomes, delivering the right care and the right time in the right place, reducing healthcare costs, and improving physician reimbursement.

Glencroft Center for the Aging – The Glencroft Center for the Aging is the largest continuum of care retirement community in Arizona. They value integrity, compassion, excellence, innovation, sustainability, and collaboration. Their services include independent living, assisted living, memory care, long-term care, and home health services. As they offer so much, they can provide a seamless transition during the aging process.

Sante Rehab of Mesa – A skilled nursing facility focused on compassionate, physician-led, highly skilled therapy services. They offer care to patients recovering from a wide range of medical conditions and surgeries, such as orthopedic, pulmonary, trauma, cardiac, neurological, renal, and wound care.
