Q: Can you describe the role of a care manager and how they collaborate with seniors and their families to ensure comprehensive support and care?

We step in as skilled advocates, trusted guides, and constant supports—so that our clients can step back into the role that matters most: being a loved one.

Whether navigating complex medical systems, coordinating in-home support, or simply being a reassuring voice during times of transition, we are here to ease the load and make aging feel less overwhelming. We like to think of ourselves as the Janes of all trades or a family member with knowledge—part problem-solver, part listener, part fierce advocate. And always, someone who shows up.

Q: How do you facilitate communication and coordination when family members live far away?

A: Whether family is across town or across the country, we ensure no one feels distant when it comes to care. We stay connected through regular check-ins, virtual meetings, and shared documentation—keeping everyone informed and involved.

Distance can bring worry, so we offer emotional support and guidance to families, providing reassurance every step of the way. As emergency contacts, we’re always ready with a plan—making sure our client’s wishes are honored. We also help caregivers care for themselves by offering resources and support for the entire care circle.

Q: What does a typical day look like for a care manager, and what are some of the key challenges and rewards of the job?

A: A day in the life of a care manager is anything but predictable. Each morning begins with a sense of adventure—no two clients, and no two days, are ever the same. We meet each person with fresh eyes and open hearts, ready to adapt and find creative solutions on the fly.

The challenges—juggling priorities, dealing with red tape, and often wishing we could be in two places at once—are real. But the rewards far outweigh them. We build deep, meaningful relationships with clients and families. We don’t just manage care—we walk alongside people in their most vulnerable moments, helping them live with dignity and, when the time comes, die with grace.

Q: In what ways has technology transformed the way care management services are provided?

A: In the 17 years I’ve been doing this work, technology has been a game-changer. Virtual tools like Zoom, FaceTime, and telehealth have bridged distances in incredible ways, allowing families to stay connected and enabling doctors to assess issues remotely—sometimes avoiding unnecessary ER visits.

Digital documentation also allows seamless communication between our team and healthcare providers. Everyone stays on the same page, no matter where they are. While technology will never replace the human connection, it empowers us to deliver care with greater precision and responsiveness.

Q: What advice would you give to seniors or families considering care management services for the first time?

A: My advice is simple: don’t wait for a crisis. Start the conversation early—when things are calm. Reach out to a few care managers and have an assessment. When your loved one has the chance to meet someone in a low-pressure setting, they’re more likely to see the value of that support.
