GUIDE Model
The U.S. Centers for Medicare & Medicaid Services (CMS) Innovation Center uses Models to test patient care strategies. The Guiding an Improved Dementia Experience (GUIDE) Model supports people with Alzheimer’s and dementia provides respite to their unpaid caregivers.
Types of care supported by this Model include bathing, grooming, companionship, meal prep, light housekeeping, laundry, medication reminders, and mental stimulation through games.

Eligibility Requirements
- Dementia diagnosis
- Medicare as primary insurance
- Traditional Medicare Part (A) and (B)
- Cannot be in a PACE or Special Needs Plan
- Cannot be enrolled in hospice
- Cannot reside in long-term nursing home
Benefits
- Calendar Year July 1 – June 30
- 80 hours of free respite care
- 4-hour shift minimum

- Delaware Palliative provides a comprehensive, team-based approach to support individuals living with serious illnesses. Each person is paired with a dedicated team who works together to address the full spectrum of care needs.
- The Penn Memory Center is a source for those age 65 and older seeking evaluation, diagnosis, treatment, information, and research related to symptoms of progressive memory loss.
PACE LIFE Program by Trinity Health and Mercy LIFE Pennsylvania
The PACE (Program of All-Inclusive Care for the Elderly) LIFE Program by Trinity Health and Mercy LIFE Pennsylvania offers a holistic approach to senior care, helping older adults remain independent and safe in their homes. The program provides a wide range of services through its adult day centers, such as primary medical care, nutrition planning, transportation, and more.

Pennsylvania Assistive Technology Foundation
Pennsylvania Assistive Technology Foundation (PATF) is a statewide nonprofit that helps people with disabilities and older Pennsylvanians acquire assistive technology (AT) devices and services to help them do the things they want to do. AT can include adapted vehicles, computers and software, tablet technology, smart home technology, adapted sports equipment, hearing aids and other devices for people who are deaf/hard of hearing, home modifications (ramps, showers, countertops), scooters and wheelchairs, seat lift chairs, and safety and security devices.
PATF provides this support through low-interest (3.75) and 0% interest loans for the purchase of AT, information and assistance about possible funding resources, and financial education for people with disabilities. PATF helps Pennsylvanians of all ages, all incomes levels, all disabilities and health conditions, and all communities and cultural backgrounds.
