At Griswold Home Care for Westminster, Broomfield, & Boulder, we believe in more than just providing phenomenal in-home care. We are dedicated to being a comprehensive support system for our clients. We want to enhance the quality of life for seniors and their families by connecting them with a wide range of community resources and services.
Local Resources:

Denver Regional Council of Governments – Through DRCOG, the Denver region’s cities, counties, and towns establish guidelines, set policy, and allocate funding for transportation and personal mobility, growth and development, and aging and disability resources. They oversee the Area Agency on Aging, of which we are a provider for their Respite and Voucher program. That program offers valuable information and referrals to trusted providers. Eligible caregivers can access respite services and receive an opportunity to recharge and take a break from caregiving duties. A one-time voucher provides up to 32 hours of in-home care to be used within 90 days from receipt of the award.

City/County of Broomfield Senior Resources – Broomfield Senior Services offers services and support for Broomfield residents aged 60 and older. Senior Services provides access to many activities for non-residents, including congregate lunches, active adult trips, recreation opportunities, and social activities. We are proud to be a provider for the city/county respite program.

TRU Community Pace – TRU PACE (Program of All-Inclusive Care for the Elderly) is a managed health care plan exclusively for older adults. TRU PACE participants receive primary medical care, medication management, physical therapy, specialty services, in-home care, social work assistance, and transportation to and from the TRU PACE day center. With TRU PACE, you can remain living in the comfort of your home and get the care you need in a safe environment.

Covenant Living – is a retirement community located in Westminster. They are a Continuing Care Retirement Community (CCRC) that offers a wellness center and a range of health care options for a confident and secure future.

Boulder Community Health – is a community-owned and- operated nonprofit health system dedicated to providing local access to high-quality medical care to people and businesses in Boulder and surrounding areas. They are known across Colorado for their clinical excellence in heart care, neurosurgery, stroke care, orthopedics, and cancer care. We’re proud of our tradition of being an early adopter of new technologies and medical techniques that improve care for our patients.
Guide Providers
Through GUIDE, every enrolled patient and their family caregiver receive all of the following at no cost:
- A comprehensive assessment and written care plan
- Caregiver training and education on dementia
- Care navigation and coordination across providers
- 24/7 access to a support line
- Up to $2,500 in annual respite care

Pocket RN – They are a Medicare-approved provider for the GUIDE dementia program. Their nurses aren’t just trained in dementia care; they’re here to listen, to guide, and to help you feel less alone. With PocketRN, expert dementia-trained nurses are just a call away, offering compassionate, personalized support as your trusted partner in care.

Ceresti – has been selected to participate in the Guiding an Improved Dementia Experience (GUIDE) Model by the Centers for Medicare & Medicaid Services (CMS) Innovation Center. The goal of the GUIDE Model is to increase access to much-needed support for people living with dementia and their caregivers.

CU Medicine – is a participating site in the Centers for Medicare & Medicaid Services (CMS) Guiding an Improved Dementia Experience (GUIDE) Model. GUIDE is a dementia care management program that supports coordinated, comprehensive care for people living with dementia.
