The Alzheimer’s Association is leading the way to end Alzheimer’s and dementia. They are the largest nonprofit funder of Alzheimer’s research, and they also focus on advocacy, care, and support. Griswold is a partner of Ceresti Health, a tech-enabled dementia care program. They work to prevent hospitalizations by sending real-time alerts to care teams. Ceresti is participating in the Guiding an Improved Dementia Experience (GUIDE) Model by the Centers for Medicare & Medicaid Services (CMS) Innovation Center. The Ceresti GUIDE Program aims to improve support for individuals with dementia and their Caregivers. Learn more here.
