A Shared Commitment to Well-Being and Equity
KHCS San Diego, Inc., doing business as Griswold Home Care for North San Diego, fulfills the Live Well San Diego partner requirements by delivering transformative in-home care and community education that empower older adults and families throughout the region vision: improving the well-being of every resident.
President and CEO April Kohnen affirmed the organization’s commitment during its Live Well Resolution Meeting on August 27, 2025, emphasizing Griswold’s dedication to person-centered, equitable, and trauma-informed care.
We are honored to stand alongside the County of San Diego and so many remarkable partners working to make this region stronger and healthier,” said Kohnen. “Our partnership with Live Well San Diego reflects our ongoing promise to uplift every client, family, and care professional we serve.
Advancing the Live Well San Diego Vision
As a partner, Griswold Home Care for North San Diego will continue advancing Live Well San Diego’s key strategic areas:
- Building a Better Service Delivery System: As California’s first Positive Approach to Care® (PAC) Designated Home Care Organization, Griswold trains every care partner in Teepa Snow’s internationally recognized dementia-care model—ensuring compassionate, relationship-centered care that preserves dignity and promotes independence.
- Supporting Positive Choices: Each care plan is tailored to clients’ strengths, cultural values, and personal preferences, empowering individuals and families to make informed decisions about daily living and long-term support.
- Pursuing Policy and Environmental Change: Griswold’s adoption of the County’s Trauma-Informed System Integration (TISI) framework helps ensure that services reflect empathy, safety, and resilience for every person touched by care.
- Promoting Equity and Inclusion: Through SAGECare approved training, Griswold ensures staff are trained in LGBTQ+ elder care cultural competency—reducing barriers, fostering affirming environments, and advancing equity for all older adults.
