April Kohnen, PAC
Co-Owner, Director of Home Care Services
April Kohnen is the Co-Owner and Director of Home Care Services for Griswold Home Care of North San Diego. A third-generation professional care partner, she grew up in a family devoted to service, with her mother and grandmother operating residential care homes for seniors and adults with developmental disabilities, eventually becoming a certified administrator. Following the birth of her daughter, Cicely, and the onset of the pandemic, she returned to the field, inspired to build a business rooted in compassion and purpose. Together with her husband, Scott, she launched Griswold’s North San Diego office, part of one of the nation’s most trusted and longest-running home care organizations, and as a Certified Independent Trainer and Coach in Teepa Snow’s Positive Approach to Care (PAC), April integrates dementia-specific expertise into the care her team provides.
Beyond her professional life, April enjoys spending time outdoors at Torrey Pines Reserve, visiting museums and art galleries, and traveling to new places.
Scott Kohnen, CFA
Co-Owner
Scott Kohnen is the Co-Owner of Griswold Home Care for North San Diego, where he combines his extensive business background with a shared commitment to service and community. Originally from southern Illinois, Scott moved to California in 2007 after completing his MBA at Duke University’s Fuqua School of Business. Scott is an associate member of the Professional Fiduciary Association of California and brings a thoughtful, values-based approach to supporting families navigating complex care decisions.
Scott holds a Bachelor of Science in Finance from the University of Illinois and is a Chartered Financial Analyst (CFA). Outside of work, Scott enjoys traveling (he’s visited over 30 countries), reading, and sharing weekend adventures with April and Cicely—especially at local museums, parks, and beaches.
Maureen Reilly, RN, PAC
Care Manager
Maureen is a registered nurse and PAC champion with experience in skilled nursing, hospital, and community health settings. She earned her Bachelor of Science in Nursing from California State University San Marcos and also holds graduate and undergraduate degrees from the University of Michigan and Emory University. With her background in both healthcare and education, she brings compassion, strong communication, and detailed care planning to her role as Care Manager. In this role, Maureen supports clients and families by carefully monitoring care, identifying changes in condition, and ensuring that each care plan is tailored to promote safety, comfort, and quality of life.
She lives in Carmel Valley with her husband and children. In her free time, she enjoys hiking, reading, spending days at the beach, and cheering on her kids at their sporting events.

Jean’s Story
In 1982, Dr. Jean Griswold founded our company at her dining room table in a small suburb of Philadelphia. She found out that an elderly woman in her community had died of complications because she had no support system for care. Jean felt this was an easily preventable tragedy and was driven to find a solution by pairing Caregivers with those who could not thrive in their homes alone. She was a true visionary, establishing one of the first and most successful non-medical home care organizations in the country, despite being wheelchair-bound due to Multiple Sclerosis. Over 40 years later, her legacy lives on nationwide.
Scholarship
Where would we be without Caregivers? Their selflessness is astounding, yet they are often overlooked and underappreciated. The Jean Griswold Foundation is dedicated to supporting Caregivers, the backbone of the care industry. We provide educational scholarships so Care Professionals can further their training, enhance their skills, elevate their careers, and better assist the people they serve.
