Each year, during the month of June, we honor Alzheimer’s & Brain Awareness Month to raise awareness of the care options available for those navigating a dementia diagnosis. Dementia does not refer to a single disease but rather to a group of symptoms associated with degenerative brain diseases like Alzheimer’s that interrupt daily life.
Alzheimer’s is the most common form of dementia, and it eventually progresses through three stages. Unfortunately, there is currently no cure for Alzheimer’s disease. However, there are resources available to help navigate a diagnosis and dementia symptoms like memory loss.
What is the CMS GUIDE Model?
The Guiding an Improved Dementia Experience (GUIDE) Model was developed by the U.S. Centers for Medicare & Medicaid Services (CMS) Innovation Center. It aims to enhance support, quality of life, and independence for people living with dementia and their Caregivers. Unfortunately, dementia care tends to be fragmented. Due to this, the GUIDE Model is testing the impact of a comprehensive Medicare-covered care package on dementia outcomes.
GUIDE was launched in July 2024 and will run until 2032. You may be identified as a potentially eligible patient, or you can reach out to your doctor to set up an assessment of your needs. Then, you can submit a form to CMS. The GUIDE Model’s standardized care delivery approach includes services like:
- 24/7 Access: Support is available around-the-clock from an interdisciplinary care team, whether it’s for a question or an urgent concern.
- Care Navigation: Participants are matched to Care Navigators. They can help access clinical services, community-based services like meals, and other resources.
- GUIDE Respite Services: CMS can reimburse GUIDE participants annually for respite care for Caregivers.

What Are GUIDE Model Participants?
GUIDE Model participants provide care and support for people with dementia through dementia care programs (DCPs). They are also Medicare Part B-enrolled providers/suppliers. We’re proud to partner with these GUIDE Model participants:
- Ceresti: Griswold is now partnering with Ceresti Health! Their tech-enabled dementia care program sends real-time alerts to your care management team in order to prevent hospitalizations. Their program includes personalized health education and care navigator coaching for Caregivers.
- Delaware Hospice: Services Delaware County through hospice care, as well as grief support. Delaware Hospice can help you navigate a new diagnosis, provide support at the end of life, or cope with a loss. Their hospice services offer a sense of dignity and quality of life for those with a serious illness that cannot be cured.
- Embrace Prevention Care: Integrates nurse practitioners, chronic disease care, pharmacy services, monitoring, and wellness through a suite of services known as Personal Prevention CareTM. Their services are available to physicians looking to add to their practice, health plans, and health systems.
- Penn Memory Center: Located at the Penn Neuroscience Center at the Perelman Center for Advanced Medicine in Philadelphia. Penn Memory’s team of physicians and clinical professionals diagnoses and treats older adults with Alzheimer’s. The team specializes in topics like cognitive neurology, geriatric medicine, nursing, social work, and more.
