Learn More About the GUIDE Program
The GUIDE Program, short for Guiding an Improved Dementia Experience, is an eight-year demonstration project from the Centers for Medicare & Medicaid Services (CMS) that began in July 2024. This is the first time Medicare has offered continuous support specifically designed for family Caregivers of individuals with dementia.
Family Caregivers are the foundation of our healthcare system, but many are overwhelmed and exhausted. Without their help, Medicare would face much higher costs from hospital stays and nursing home care. That’s why CMS created the GUIDE program to finally provide Caregivers the support they need.
GUIDE puts families first, especially those caring for loved ones with dementia. This program gives you direct access to a team of support professionals, a 24/7 helpline, Caregiver education, and additional respite hours, all at no cost. GUIDE respite hours deliver a lifeline during transitions to memory care, providing essential one-on-one help in those challenging early weeks. By removing cost barriers, GUIDE helps families get quality home care exactly when they need it most.
GUIDE Program Benefits
GUIDE is a complete care coordination program. Every participant and their family Caregiver is eligible for all of these benefits at no out-of-pocket cost:
- A comprehensive assessment and written care plan
- Caregiver training and education on dementia
- Care navigation and coordination across providers
- 24/7 access to a support line
- Up to $2,500 in annual respite care
How Providers and Agencies Collaborate
A large number of individuals with dementia remain undiagnosed. After a formal diagnosis is made for a Baltimore senior, the GUIDE program offers essential assistance. Our team manages the referral process, connects families with a specialized care management group, and maintains close contact with local healthcare providers. What Qualifies vs. Does Not Qualify

Can GUIDE Work with Other Funding Sources?
Absolutely. GUIDE respite dollars add to your existing support, no matter what funding your family already receives. GUIDE provides extra hours without taking any away.
- LTCI: GUIDE dollars do not count against your policy or draw it down faster. Many families forget they even have a policy.
- VA (Aid & Attendance / HH Aide): VA authorizations and GUIDE respite are two independent funding streams for the same client. We coordinate scheduling to avoid overlap.
- Medicaid / other benefits: If another program is funding care, GUIDE adds to it. GUIDE is a separate Medicare benefit. It adds up to $2,500 on top of what you already have, at no cost.
